A bolus calculator applies your personal ICR and ISF values to a standard formula. The result is only as accurate as the values you enter. The formula does not account for: active insulin on board (IOB), glycemic index, physical activity, illness or infection, stress hormones, or alcohol. Always treat the result as a starting point for discussion with your diabetes care team.
Both measure blood glucose concentration. mmol/L is used in Europe, Canada, Australia and Asia. mg/dL is used in the United States. To convert: multiply mmol/L × 18.016 = mg/dL. Example: 5.5 mmol/L = 99 mg/dL. Use the toggle at the top of the calculator screen to switch units.
This calculator uses the standard ICR/ISF bolus formula designed for basal-bolus insulin regimens — most commonly Type 1 diabetes and insulin-dependent Type 2 diabetes. If your doctor has provided ICR and ISF values, it may be useful. Always consult your diabetes care team first.
